01 The clinical difference

SPECT can miss it. PET catches it.

Absolute quantitation of myocardial blood flow (AQMBF) is the differentiated capability of cardiac PET. Where a traditional SPECT exam reports relative differences between walls, PET reports actual blood flow values in mL/min/g — surfacing balanced multi-vessel disease that SPECT routinely reads as normal.

The clinical implication: when your patient’s symptoms point to disease but their SPECT reads normal, PET with AQMBF resolves the diagnosis. We perform every PET study with quantitative flow analysis as the default — not an optional add-on.

04 Common indications

When cardiac PET changes the answer.

Cardiac PET is the right modality when SPECT image quality will be degraded, when balanced disease is suspected, when microvascular dysfunction is in the differential, or when quantitative flow data will change clinical management.

  • Inconclusive treadmill or SPECT in a symptomatic patient
  • BMI greater than 30 — SPECT image quality degraded by attenuation
  • Female patients with significant breast tissue or implants — SPECT artifact
  • Diabetes — high silent ischemia risk, balanced disease pattern
  • Microvascular disease suspected in symptomatic patient with normal angiography
  • Pre-operative cardiac clearance when treadmill testing isn’t feasible
  • Post-stent or post-CABG functional assessment
  • Known CAD where flow quantification will guide medical vs. interventional management
  • Heart failure workup — ischemic vs. non-ischemic etiology
